I'm not quite sure I understand the ending... Is the message that child abuse breeds maniacs? That some day he grows up to fear 'exiting' because he associates a bad memory with birth (not that babies have a developed long term memory).
Is the point supposed to be that the whole time he's actually in the womb? I guess that would make sense. Given that metaphor the 'water breaking' would make sense... but generally doctors aren't personally responsible for that.
I guess I just have trouble understanding the message, which these kinds of things very frequently have in some way or shape.
Regardless of the message, I like the doctor's character design.
